ADHD Mind (adhdmind.ie) is operated by Revival Point Ltd (CRO 654924), Fairlands, Ballinacurra Road, Limerick, Ireland, trading as ADHD Mind, a Revival Mind service. ADHD Mind is a structured self-report screening service. It administers validated screening questionnaires with their published scoring rules and generates a personalised report designed to support a conversation with a GP or an assessing clinician. It is not a diagnosis, not a medical device, not medical advice, and no clinician reviews your individual results. Only a qualified clinician can diagnose ADHD or any other condition. Never delay or disregard professional medical advice because of anything in this service. Adults aged 18 and over. The screening instruments and the information pages are written for adults; children's screening works differently and belongs with your GP.

The screening is free to take, and so is the result you see on screen, and so is the full report, sent by email. The Assessment Ready Pack (the two-page summary, your scripts, the appointment and childhood evidence pages, the Ireland assessment guide and the ebook) costs the price shown at checkout (€19 at the time of writing), including applicable VAT. The standalone ebook costs €6, with no screening or personalised report included. Its download link is available for 12 months, and downloaded copies can be kept. Payment is processed by Stripe; your card statement will show our descriptor (currently REVIVALMINDADHD). Reports and packs are digital content delivered by email link within minutes. Links expire after 30 days, or after 12 months with the Assessment Ready Pack. By purchasing, you request immediate delivery of digital content and acknowledge that, once your pack has been generated and delivered, the standard 14-day EU withdrawal right no longer applies to it. Our own guarantee goes further: if the pack is not useful to you, email hello@adhdmind.ie within 30 days of buying it and you get a full refund, even after you have downloaded it. Separately, and as the law requires, if anything you paid for does not arrive or will not open, you are entitled to have it put right or refunded.

The preparation tool organises your own words. It does not interpret symptoms, assess you or guarantee a referral or a particular response from a clinician. The tool itself is free to use, and it is not sold separately. Its PDF downloads, the Ireland assessment guide and the cost worksheets are part of the €19 pack, and pack buyers can use them for the remaining life of their pack link. Drafts stay in the browser where they are written; a purchase link does not restore notes from another device. Downloaded files are yours to keep. If access or downloads fail, contact hello@adhdmind.ie for help or a refund. Your statutory rights are unaffected.

The value of your report depends on answering honestly; the scoring applies published rules to whatever you enter. If you use an Assessment Ready Pack informant link, choose the recipient yourself and choose carefully: their words appear in your appendix verbatim, and sending them the link is your decision. How we handle all data is set out in the privacy policy, which forms part of these terms.

You may download, print, and share your report and appendix with whoever you choose, including your GP and any clinician. The service's content, design and question presentation otherwise remain the property of Revival Point Ltd and the respective instrument copyright holders, whose attributions appear where their instruments are used.

Nothing in these terms excludes liability that cannot be excluded under Irish law. Subject to that, our total liability to you in connection with the service is limited to the amount you paid us, and we are not liable for decisions taken on the basis of a screening result without appropriate professional advice; the service exists precisely to route you toward that advice.

These terms are governed by Irish law, and Irish courts have jurisdiction; nothing removes protections you hold as an EU consumer. If we change these terms, the new version appears here with a new date and applies to purchases made after it. Effective date: 28 September 2026.
